The following files need to be printed, signed and returned by January 15th. Students need to have the following documents on file to be eligible to play; Physical Form (annually submitted), Concussion Form (annually submitted), Insurance Form, AHSAA Student Release Form (annually submitted), Birth Certificate, Star Sportsmanship.
The following file needs to be printed, signed and returned by January 15, 2016. This is the Golf Sponsorship Form. Each player will need $150 to purchase uniforms. I have also included this years welcome letter with inmportant information!
